Participant Information

Early Registration: Anyone registering before July 1st will receive a camp t-shirt! Those who register after this date may not receive requested size.

Medical Information/Emergency Contact

In the box below, please list any other medical conditions that Camp Staff should be aware of for the well-being of your child (i.e. allergies, asthma, diabetes, epilepsy, heart problems, previous concussions, recent injuries); or check none.

The EpiPen must be kept with the participant at all times, and checked for expiration dates with camp staff. If the participant is exhibiting symptoms of anaphylactic shock and is unable to administer the EpiPen on their own, a staff member will administer the EpiPen and contact EMS. In order to participate in camp, the parent or guardian of the participant must consent to this policy.

Disclaimer and Consent of Risks Agreement

The King’s University is not to be held responsible for any participant’s death, injury, loss or damage of any kind sustained by any person while registered as a participant of King's U-Camps, except to the extent that such injury, loss or damage was caused by negligence.

In consideration of my child’s participation in King's U-Camps and all related activities (i.e. movement skills including: agility, balance, coordination, speed, strength, endurance, flexibility, etc), I and my child acknowledge that we are aware of, appreciate and accept the inherent physical risks and other possible risks, dangers and hazards associated with being a participant, including the possible risk of sever or fatal injury to my child or others. These risks include, but are not limited to:

  • All manner of injuries resulting in muscular injuries and soft tissue injuries including bruises, scrapes, cuts, etc, from executing strenuous and physically demanding physical techniques, collisions with the wall, floor, uneven playing surfaces, contact with other participants and failure in proper use of equipment either by my child, or other participants;
  • All manner of injuries resulting from the mechanical failure of apparatus/equipment;
  • All manner of injuries resulting in dislocations, concussion, hematomas, whiplash, contusions, sprains, pulled or strained muscles, knee injuries, and broken bones;
  • Transmission of diseases through contact with camp staff or other participants resulting in death, disease or other illnesses;
  • All manner of head, neck, spinal, facial, eye, nose and/or dental injuries;
  • That my child’s risk of injury increases as they become fatigued

Participant Photo & Media Release

I hereby grant permission to The King’s University to use my child’s photograph for promotional materials including websites, social media, brochures and other educational publications.

Privacy Protection

The personal information requested on this form is collected and protected under the authority of the Personal Information Protection Act (PIPA), and will be used for the specific purpose of administrative management only.

Cancellation Policy

All cancellations will be subject to a $20 administration fee.
No refunds will be given after midnight of June 23, 2024 with the exception of a family emergency.
