Exam Invigilation

Before completing the form below please read through the following points to determine your eligibility for special exam invigilation:

Student must be registered with the Student Support and Accessibility Office prior to receiving exam accommodations.
Be sure that you have spoken to each professor prior to the test in order to remind her/him of the accommodations you have been granted.
Student must submit completed form to the Student Support and Accessibility Office TWO WEEKS prior to scheduled exam, or THREE WEEKS prior to the first day of final exams.
Any special requests must be discussed with the Student Support and Accessibility Office in the Centre for Teaching and Learning (CLT).
